Requesting a Copy of your Medical Records
At Melbourne Podiatrists & Orthotics we provide patients with two options when it comes to obtaining a copy of your medical records.
OPTION 1 – Full copy of medical records
Fees apply
Request a full copy of all medical records including clinical notes, all attachments, radiology reports, orthotic scripts and other relevant information. We are entitled to charge a fee for this service.
OPTION 2 – Complimentary one-page summary letter
Patient requests only
Request a one-page summary letter of your treatment (the most important information) at Melbourne Podiatrists & Orthotics. It will also include any other current and relevant information (e.g. radiology reports, orthotic scripts) that may assist another treating health professional. This is a complimentary service we provide free of charge.

Requesting a Full Copy of your Medical Records
Current fee: $60 (plus GST for all third-party requests)
NB. Fees are current of 1st July 2025 and are subject to price increases on the 1st July each year.
Formal request process
1. Firstly you will need to provide us with a formal request by completing and returning the form below:

NB. Melbourne Podiatrists & Orthotics is legally allowed up to 45 days from the receipt of a written request to provide medical records. However, in most cases, we aim to process requests within 2 weeks.
2. After the request form is received you will be sent an invoice for payment.
3. Once payment is received your medical records will be delivered via secure, password-protected link to your nominated email address.
Requesting a One-Page Summary Letter
No fee. Complimentary service.
Formal request process
1. Firstly you will need to provide us with a formal request by completing and returning the form below:

NB. In most cases, we aim to process requests within 2 weeks.
2. After the request form is received your main treating podiatrist (or another podiatrist if they are unavailable), will write a one-page treatment summary letter. It will also include any other current and relevant information (e.g. radiology reports, orthotic scripts) that may assist another treating health professional.
3. The one-page summary letter and any other current and relevant information will be delivered to your nominated email address.
